This care card's answer
There needs to be a real assessment.
A form offering only fixed answers cannot be the sole basis for treatment, according to Federation of State Medical Boards guidance checked on September 26, 2026.
This group of state bodies that license doctors says an online interview must let the health professional ask more questions or learn more about your health.
Put your questions beside your health notes.
Have the names of your medicines and supplements to hand. Add past health problems, allergies and any questions about pregnancy or breastfeeding that apply to you.
Use your own words to describe the problem to the clinician. A short note is fine. Ask them to explain any term that makes the answer harder to follow.
Ask what happens if more is needed.
Find out how to send missing information, ask another question or arrange a test. If the reply is unclear, ask for it in writing.
Take this to the conversation
Who reads my form, can they ask more questions, and how can I ask mine?
Ask for the answer in plain words. Keep it with your visit notes.
